River Song first meets The Doctor in the silent library season 4 episode 8.
But there is much more to it. And im not gonna do any spoilers.
But for you who wants to cut straight to the real deal about this little blue book and why it all over sudden have a red ribbon around it, have a look at Lets kill Hitler Season 6 episode 8.

My wife said it just had to be done !. And i couldnt agree more. This is an unique Doctor Who item. I just had to MOC it !! :p
This buid was done in about 2 hours. But i couldnt finish it properly and make it the way i really wanted it, again, cause i didnt have the right blocks.
But i kinda like it as is. :)
